What is CAP/DA?

The Community Alternatives Program for Disabled Adults (CAP/DA) is a cost-effective home and community-based program designed to assist families in caring for medically-disabled adults instead of placing them in a nursing facility.

Available CAP services include:

  • Case management
  • In-home aide
  • Respite care
  • Consumer directed services
  • Coordinated caregiving services
  • Personal Emergency Response Services (PERS)
  • Specialized medical supplies
  • Equipment, modification and technology
  • And others

Who is eligible for CAP/DA?

To be eligible, you must:

  • Be 18 years of age or older 
  • Be disabled and have an intermediate or skilled level of care requirement
  • Be eligible for Medicaid
  • Live in a private residence and be at-risk for institutionalization or live in a nursing facility and wish to return to a private residence
  • Need at least one or more CAP/DA home- and community-based services as listed above

How to Make a Referral

Referrals are accepted from anyone having knowledge of the person. There are two ways you can submit a referral:

CAP/C

The Chatham County Public Health Department does not offer Community Alternatives Program for Children (CAP/C) services. CAP/C services are offered in Chatham County through Amazing Kids of NC, Inc. (919-435-6970), Walker’s Home Care (252-432-6308), RBS Case Management (910-206-7974), and RHA Health Services, Inc. (888-207-7828). These agencies provide long-term, home-based nursing and other medically necessary services, supplies, and equipment to children with special healthcare needs who live at home with their family. Call those numbers for more information.
