REMOVE DECEASED VOTER:  To remove a voter from the voter registration list that has been deceased. The Board of Elections office can only remove deceased voters with a written authorization from a near relative OR information from the Department of Health and Human Services OR a death certificate. To remove a deceased voter please complete required NOTIFICATION OF DECEASED VOTER FORM

CANCELLATION OF VOTER REGISTRATION: This form is intended to provide notification of a voter's request to cancel his or her voter registration.  This form can only be completed by the voter and must be signed.  If you would like to request your voter registration be cancelled please complete required CANCELLATION OF VOTER REGISTRATION FORM

VOTER RECORDS REQUEST:  The Board of Elections Office provides copies of lists of public records information as requested. To request information please complete required VOTER RECORDS REQUEST FORM

VOTER REGISTRATION FORM:  To register as new voter or make changes to  an existing voter you must complete a required NORTH CAROLINA VOTER REGISTRATION APPLICATION
